The Counseling Center for Change (“we,” “our,” or “us”) operates www.tcc4change.com (the “Site”). This Privacy Policy explains how we collect, use, and protect information through the Site, including information submitted through our contact and program interest forms.
We also maintain a separate Notice of Privacy Practices that describes how protected health information may be used and disclosed and explains applicable patient rights. This Privacy Policy does not replace that notice.

Contact information: Your first name, last name, email address, and any other contact details requested by a form.
Inquiry information: The subject of your inquiry and anything you choose to write in a message field.
Program interest information: Answers to questions on forms such as the Bariatric Success Blueprint Interest Form, including whether you are a pre-bariatric surgery patient and whether you have Maryland health insurance.
Communication preferences: Your choice about receiving practice news and updates, where that option is offered.
The Site and its service providers may also collect technical information needed to operate and secure the Site, such as IP address, browser type, device information, and visit or submission records.
3. How We Use Information
We use information collected through the Site to respond to inquiries; follow up about requested services or programs; discuss program fit, scheduling, or insurance questions; coordinate services or events; operate and protect the Site; and send practice news or updates when you have chosen to receive them.
Submitting a website form does not establish a patient-provider relationship or guarantee eligibility for a program or insurance coverage. When information is protected health information, we handle it in accordance with applicable law and our Notice of Privacy Practices.

We do not sell information submitted through our website forms or share it with third parties for their own marketing.
Service providers that help us host the Site, manage forms, or communicate with you may process information on our behalf. We may also disclose information when required or permitted by law. The Site is not directed to children under 13. We do not knowingly collect personal information through the Site from children under 13. If you believe a child under 13 submitted information through the Site, please contact us.
